USB Drive not mounting and unmounting right

Ok Im lost, I have been trying to make this system work right. My USB drive will not "reset" I have partitioned it and loaded an ISO onto it. But everytime I put it in the system tells me its the old system / files.

I have an IBM Thinkpad T60 (older model) Its running Linux mint 14 / Win XP.

Ive used gparted and partitionmanger both cause the same issue.

This is the error i have been getting after I have "erased the drive" I left it unpartitioned and restarted the computer. Still get this error.

In some cases useful info is found in syslog - try
dmesg | tail or so

Any Ideas?

I didnt need to. Popped it into my desktop linux box and now it shows it as a 32 GB USB drive. Well I know its only a 4. So I popped it into my Win7 computer and formated it. Windows reported 1.74 GB. The drive it self is old so It probably failed.
