[Errno 5] Input/Output Error

I have attempted I don't know how many times to install Mint 13 KDE with zero luck. I've tried everything from turning off apic on boot to installing from DVD/thumb drive, to manually running the installer from the command line with root priveledges. Here's the debug log from the last installer attempt:
I couldn't find anything relevent in any other log files.
