scan to PDF and create form - Typewriter

I want to scan in a paper form and turn it into a PDF form, so I can fill it out, like I was using a typewriter.
It looks like a windows software from Nuance, PDF Converter Professional 6, does this. Is there a Linux GPL that does this?
I tried PDFedit, but could not get it to work, so far.

I'll admit this is a little off the wall but if your question is how to fill out the form after you have the pdf you might try this:
Install xournal from synaptic
Open xournal: Menu > Accessories > Xournal
Once opened select File > Annotate PDF
Then Tools > Text ( you can change the text font and size: Tools > Text Font )
When you're done you can File > Export to PDF
The placement of the inserted text can become nerve racking, and if you have a large form with many inserts I guarantee you will search the net for a better alternative. But for small jobs it might be worth a look.
