Just tinkered, and given your picture, you're running off of the live .iso, and not an installed system. Running off the iso, you can't run the guest additions image because the optical drive already has the lmde6 iso loaded. Instance of two items trying to occupy the same space, it ain't happenin'....

As gm10 pointed out, you only mention installing the linux-header files.

If not already done, you still need to install the actual kernel file linux-image-4.19.0-0.bpo.4-amd64. Once you install that and update grub, you should see the new kernel listed.

Edit: Oops, just noticed that you have ntfs on a primary partition, and / in the extended partition. As mentioned before you'll need to use gparted from a live dvd/usb, since your Linux build lives within the the extended partition and things need to be unmounted in order to make the change. ------ ...
