looking at your before and after I noticed some strange things and tend to point to windows did access the Linux files (did you actually boot windows between al the tries you made to install?) http://www.rodsbooks.com/linux-fs-code/ I guess the best would be to manually recreate the linux partitions...

that is definetely not the correct driver.
I have the same card and Mint17
just had to install fglrx-updates using driver manager and now I can select witch card to run.
witch driver did you download from AMD?

just checked it with the UUID it did not work however I used the dev and that did work

it actually gave back the control to windows, so your partitions may still be there but it won't allow them to be seen.
Also make sure that you install either both in EFI mode or mbr mode, don't mix or Mint updates or Linux updates could break the installation again.

never a good idea to run everything as root (even windows restricts it now a bit more) also I just did an apt-get dist-upgrade on a VM I'm running as test unit, you will see all the harm it can do if not applied carefully. secure boot
that is probably the main reason as it disables any "non signed code" being installed or run (it's a table that the manufacturer provides, so if only windows is mentioned you can imagine what it will do to Linux).

did you actually enable the grubline
GRUB_CMDLINE_LINUX_DEFAULT="nosplash enable_mtrr_cleanup=1 resume=/dev/sda5"
check you have the correct partition for swap.

nope that's bad enough something in the upgrade broke the link between the driver, kernel and probably xorg.
Tru the drivers that says 331-updates and let's hope it does correct itself.

sudo apt-get fglxr
is only the first part once it's done you need to configure it.

that's why it's recommended to use the driver manager to do the updates for ATI or nvidia.

did you try uninstalling mdm and installing again?
