From my experience, there are a few method of installing AMD proprietary driver. 1. distro method. NVIDIA NV17M is legacy.
Support of drive stopped sometime ago.

The latest version that should still support proprietary driver should be Linuxmint 13.

You may have RAID data left over in the hard disk.

Try using this command to remove the raid meta-data

What kind of backup you have?

The screen shot said so too, your sda5 is now 250M size.

The partition table have been corrupted.

I think it take less time to reinstall , either from fresh, or restore from backup.
