Rek: when you get the desktop with the infinity symbol button, you're logging into the default GNOME session which uses Gnome Shell as desktop environment. Press the windows key to open the overview, write a+l+a+c+a... until the program "Main Menu" appears. Open it. In it you can check/uncheck which icons you want to be visible. My guess is on your system Sound Recorder is checked twice in two different categories and you need to disable one of them. H...

With alacarte (aka Main Menu) - can't remember if it needs to be installed (sudo apt-get install alacarte) or if it's there by default.

Same here, Faenza is one the first things I have to install whenever I got a new ubuntu/mint/pinguy to play with. That, Elementary and Awoken are the only 3 really good icon themes for Linux. They're easily installed too.
