WiFi Requested A Pair of Username and Password

The WiFi connection of my newly installed Linux Mint 18.3 Cinnamon works in my home where a WiFi password is requested to access the wireless connection. It, however, doesn't work in my workplace which its WiFi requests a pair of username and password. Under the security tag, there are quite a few options:
  • WEP 40/128-bit key (Hex or ASCII)
    WEP 128-bit Passphrase
    LEAD
    Dynamic WEP
    WPA & WPA 2 Personal
    WPA & WPA 2 Enterprise
I enter a pair of correct username and password under the LEAD. That, however, doesn't work.

What is a right setup?

You are asking how your workplace has protected its Wi-Fi network; should as such primarily ask them. That said, chances are it's WPA2 Enterprise with PEAP and MSCHAPv2.
