Authentication not working Proximus Smart wifi

Hello everyone,

i am using mint 18.1 (serena) and
Since a few moment i cannot connect anymore on proximus Fon : no connection page appearing anymore.
so i tried to connect to the proximus smart wifi but the connection windows appeared (see image) and i really don't know wich parameter had to be used...

i tried to connect with a windows 10 computer, and all works perfectly, so i get the connection parameters, but it is not enough to be able to connect.

I tried to phone proximus but they don't support Linux ...

anyone had already this problem or know witch paremeter is correct ?
