Onion - usually you have to ask for onion and or peppers. Then there is "all the way" onions, peppers, lettuce and tomato.
Whenever I go back home to Philly I go "all the way" plus catsup and mayo. And yeah, it's definitely artery clogging but as the saying goes;
"If you like it then it is probably fating, illegal or immoral."
