If I'm picking up on this as intended, it is, and it actually happened on a set if I remember correctly.

No actors actually died shooting the the Russian roulette scene.
It could have happened if the prop masters made a mishap.
However a mishap just happened.

I suppose the prop master was licensed by New Mexico. The location of the set for the movie.
Sometimes even master shooters need master to double and triple to check each other. With all those safe guards in place it is possible to get complacent and say the guy before me said its safe, so it must be safe. Never under estimate human fallibility.
I wonder how many, if any, actors get fire arms training before the handle a gun, prop or real, before they "shoot" a scene involving fire arms?

The last one to check the fire arm in a movie scene should be the intended victim of the scene.
