So my desktop has three monitors- a 4:3, a 5:4, and a 16:10. Before I installed Mint 21, the 4:3 was listed as Monitor 1, the 5:4 as Monitor 2, and the 16:10 as Monitor 3 across all applications. That changed shortly after I installed Mint 21 and now it's all messed up. In the Display window, they are the same as they were before. However, if I right-click on a window in the panel and select "Move to Monitor 1," the window moves to the 16:10 (monitor 3). Selecting "Move to Monitor 3" moves the window to the 4:3 (monitor 1).
With all the changes made so far, you are still getting what I will call the "mismatch" with number of monitors 1 and 3 and only those two? Or did the numbers change on the Display app, but the "move to Monitor x" part is still wrong? Just trying to get an idea if any of the changes we made had at least a partial effect.
When reading xrandr output,
*
= current resolution and
+
= preferred resolution. I think I labeled these correctly. It seems the two monitors with 60Hz are what are switching. According to xrandr, you are using Monitor 2 at 75Hz, but the system sees the preferred refresh rate as 60Hz.

It appears you still have several LM20 (focal) repos installed. I am not familiar with these ones so I do not know what impact they might have on the system.

When you had the amdgpu pro drivers installed in the past, did you run the uninstall program? I see the repo is not active and the drivers indicate they are coming from the kernel, but just checking what might be a possible issue.
